*Identify State Agency Specific **Remote Gateway-to-Remote Gateway VPN Concentrator software configuration and installation cost associated with the remote ends (far end meshed tunnels) of the LAN-to-LAN connection. ***Please complete and submit a CSA for any service changes, i.e., for New Accounts, Changes to Accounts and/or Disconnection of Accounts. ****When a customer with Hayes' owned equipment disconnects their service, the equipment needs to be returned back to the vendor. Billing will continue until the Hayes' owned equipment is returned. The official procedure is that Hayes will issue a UPS call tag, and UPS will pick up the equipment at the customer site. Hayes will call the customer the day before UPS is scheduled to pick up the equipment. *****External contractors requiring access to VPN services need to have their sponsoring state agency to submit CSA orders to DMS SUNCOM for the VPN account(s). External contractors have the option to have the vendor (Hayes) directly bill for the VPN monthly recurring fees at $20 per user account. | ||||
