Recertification Information and Affidavit
By law, the OSD grants certification for a period of one year. Businesses interested in maintaining their certified status must submit the recertification affidavit to the OSD on or before the expiration of their current certification.
The affidavit must be filled out completely, signed, notarized and mailed to the OSD.
