Certification Documentation Requirements
The documents required for CBE certification varies by business structure, length of time in business and ethnic category. Please submit the items to the Office of Supplier Diversity within 25 days of completing the MyFloridaMarketPlace registration so that your CBE certification can be completely processed.Please call (850) 487-0915 or email OSDHELP with any questions you may have.

PLEASE NOTE
- Faxed or emailed documents will not be accepted. Please mail all documents to the office address listed above. As a recommendation send documents via a mailing service that provides a tracking number.
- We DO NOT certify non-profit corporations.
- Business Owners MUST be U.S. Citizens in order to qualify for certification.
- Resident Aliens DO NOT qualify for certification.
- Only send the documents listed on the checklist. Please do not send social security cards, personal bank statements, etc.
