Forms

The Division often revises many of its frequently used FRS forms to make it easier for you to do business with us. Please discard any out-of-date forms you may have and begin using the revised forms available below. For various reasons, some Division forms are not available on our Web site; however, they are available on request from the Division. Completed forms may be mailed or faxed to the Division. ( If you fax us a signed and/or notarized form you must retain the original form. You do not need to mail the original to us, unless requested.)
Many of the forms on this Web site are fill-in forms (except the Direct Deposit Authorization form for retirees - a form created by the Office of the Comptroller - and enrollment forms created by the Florida State Board of Administration). With fill-in forms, you can enter your information while the form is displayed by an Adobe Acrobat 4.0 (or later) product and then print the completed form out. You can view our fill-in forms summary sheet to get more information on using fill-in forms.
We have an Alphabetical List of all our forms available electronically on our Web site. However, if you do not know which form you need you can search our Categorical List of forms.

Tips

  1. These forms will print best on a laser or other high-resolution printer. If printing on a non-laser printer, choose the highest dots-per-inch (dpi) setting available for your printer.
  2. If the PDF doesn't display, try turning off your pop-up blocker.
  3. Select the "Fit to Page", "Shrink to Fit", or other similar choice within Acrobat Reader's print settings to ensure proper sizing of the form during printing.
  4. When completing fill-in forms online, type slashes (/) in date fields where they are not provided for you.
