Title:Uniforms
Two changes have made in the above referenced contract as a result of this Amendment # 4:
·The Contract Award for Lot # 1, (Uniform products for the Florida Department of Highway Safety and Motor Vehicles – Division of the Florida Highway Patrol) and Lot # 5 (Uniform products for the Florida Department of Environmental Protection – Division of Parks and Recreation) have been withdrawn from Superior Uniform Group
·In accordance with rule 60A-1.006(3)(c), F.A.C., these same lots and Uniform products (Lots #1 and 5) are awarded to VF Imageware.
As VF Imagewear produced the second-highest scored Request for Proposals response for the Department’s RFP # 32-200-050-O, has agreed to abide by the Prices, Terms and Conditions set forth in their RFP response, and has signed the Contract Document acknowledging they have been added as a supplier to State Term Contract # 200-050-05-1 as a result of Amendment # 2, they are hereby awarded Lots # 1 and 5 of this Contract.Likewise, Superior Uniform Group’s Contract Award for Lots # 1 and 5 are terminated.
