Title:Uniforms
Two changes have made in the above referenced contract as a result of this Amendment # 2:
·The Contract Award for Lot # 10, (Uniform products for the Florida Department of Environmental Protection – Division of Law Enforcement) has been withdrawn from Superior Uniform Group
·In accordance with rule 60A-1.006(3)(c), F.A.C., this same lot and Uniform products (Lot #10) is awarded to VF Imageware, and VF Imagewear has been so added as a supplier on this State Term Contract
As VF Imagewear produced the second-highest scored Request for Proposals response for the Department’s RFP # 32-200-050-O, has agreed to abide by the Prices, Terms and Conditions set forth in their RFP response, and has signed the Contract Document acknowledging they have been added as a supplier to State Term Contract # 200-050-05-1, they are hereby awarded Lot # 10 of this Contract.Likewise, Superior Uniform Group’s Contract Award for Lot # 10 is terminated.
