The following changes have been made to the subject contract:

Nutricia:

  • One new Periflex product has been added to the contract from Nutricia. Commodity number and names: 388-600-815-1933: Periflex Infant, Unflavored.
  • Commodity number 388-600-815-1932 name correction from Periflex Advance to Periflex Junior.
  • Ordering account statement for Dept. of Health Customers added to the Ordering Instructions. Only product greater than 6 month expiration will be accepted for the Department of Health. See Ordering Instruction on this contract.
  • Nutricia Balance of line price list updated.

McKesson:

  • Balance-of-line link removed - no discount or price list provided.

Mead Johnson:

  • Two products discontinued and removed from contract: Kindercal Tube Feeding, Commodity Number 388-600-542-3600 and Kindercal Tube Feeding with Fiber, Commodity Number 388-600-542-3610
  • Balance of Line price list items updated with no prices. Please contact Mead Johnson for current prices.
