Title: Dietary Supplements & Vegetable Protein Dinners
The following changes have been made to the subject contract:
Nutricia:
· Please note: current Periflex products from Nutricia (Commodity #’s: 388-600-815-1900 and 388-600-815-1910) are discontinued and inventory is no longer available.
· Three new Periflex products have been added to the contract from Nutricia. Commodity numbers and names:
o 388-600-815-1930: Periflex Junior, Unflavored
o 388-600-815-1931: Periflex Junior, Chocolate
o 388-600-815-1932: Periflex Junior, Orange Flavor
· Commodity number 388-600-815-1790 has had a name change from Pediatric EO28 to EO28 Splash.
· Commodity number 388-600-815-1830 has changed amount from 10/100 grams to 15/60 grams. (See price sheet for corresponding price change)
Mead Johnson:
· Commodity number 388-600-542-900 (balance of line) has changed from 3 oz bottles to 2 oz bottles.
· Commodity number 388-600-542-2040 (balance of line) has changed from 3 oz bottles to 2 oz bottles.
· Commodity number 388-600-542-2200 (balance of line) has changed from 3 oz bottles to 2 oz bottles.
· Commodity number 388-600-542-2416 has changed from a single-hole nipple to a standard flow soft nipple.

Novartis:
