Title: Dietary Supplements & Vegetable Protein Dinners
The following changes have been made to the subject contract:
· Please note: current Periflex products from Nutricia (Commodity #'s: 388-600-815-1900 and 388-600-815-1910) are being discontinued and inventory will be available until June 1, 2007. After June, Periflex Junior will replace Periflex (original on contract) with an anticipated price increase.
· Three new Periflex products have been added to the contract from Nutricia. Commodity numbers and names:
388-600-815-1920: Periflex Advance, Unflavored
388-600-815-1921: Periflex Advance, Chocolate
388-600-815-1922: Periflex Advance, Orange Flavor
· Enfamil® Single Hole Nipples Mead Johnson commodity #388-600-542-2416 has been discontinued and removed from contract.
· Novartis has submitted an update Balance-of-line price sheet.
