(1) Lamps, which are accidentally broken during handling, packing or transport.  
(2) Fluorescent lamps that are intentionally crushed into a drum using drum-top crushing equipment per 62-737(6)(b), F.A.C.
(3) Mercury containing devices shall include but is not limited to thermostats, switches, thermometers, medical and dental tools and devices.
             
ADDITIONAL PICK UP TIME: Will be billed at the rate of: $50.00  
          per 30 minute period.  
MINIMUM ORDER: Minimum order on this contract shall be: $150.00  
QUANTITY DISCOUNT: Additional discount to be deducted from net prices for large single orders of assorted items, from (1) location.
